probably hacked website, you need to check the logs are clean account from the malware

This could be a serious issue and should be done properly by the experienced sysadmin or cwp support from the start or the malware will expand and it will be in the backups and you will not be able to recover that site anymore ...or the only way will be to remove all files and install all new.

Also if you don't have cwp secure kernel that kind of malware could expand on other accounts.

probably, you should check grants for it.
It's always recommended to keep MySQL port 3306 closed in the CSF firewall to avoid remote connections.

you will need to check the logs and config, probably you have issues with IP settings for shared IP or vhost

mostly with any provider, the port is open. Only if you have some NAT networking then you could have an issue and you would need to do that on the server provider management tool.

Also, there is cwp support where you can check issues with the support and you can do that also with one-time support which is like 7USD....but you have probably some other issue and not port locked.

you have info about this in csf firewall, as using big block lists with firewall will slow down the server response time as it need to check each IP.
