Cannot creat new user sql

My sever info:
Apache version: Apache/2.2.27
PHP version: 5.6.14
MySQL version: 5.6.30
CWP version: 0.9.8.14
I have problem with user. Can't creat new user to add to database. After create, notice creat success but not show after

Create database and user from user account not from root user account. You need to login as user to create database and username.

I have the same problem but only with long usernames. The username has 16 characters . If i try the same with shorter usernames
10 characters for example it works as expected...
