CentOS 6 Problems / Very high RAM usage, resulting in failure of server

Hello I have install CWP on my server (configuration mentioned below). As the days progress the Memory keeps on increasing and in 5-6 days it reaches to 99% and CWP will hang and on my vmware vSphere client I get error - the CPU has been disabled by the guest operating system. And I have to reset the VM.
This is the 5th I have faced this problem in 1.5 months period, Please let me know how can I resolve this problem.

Server Time: Fri Feb 6 10:32:21 IST 2015
• Memory RAM: 3.13GB / 6GB ( 52.2%)
• Swap Memory: 0GB / 4GB ( 0% )
This is the 5th I have faced this problem in 1.5 months period, Please let me know how can I resolve this problem.
