CentOS 6 Problems / sd 2:0:0:0 rejecting I/O tı offline device

I have a problem with my HP Proliant G6 server.
Alert is sd 2:0:0:0 rejecting I/O to offline device.
Then not do anything until I reset webserver
It is happen every 8 days after
Please help me
