Cannot create db user

New GUI for mysql manager.   uggg.  What was wrong with the old one?

New GUI create user is a FAIL.

New user fails for root and for users.

I have three servers with rel .1095, one is working 2 are not.

working:

php 7.1.11 mariadb 10.2.39 , cent 7.9

NOT working:

php 7.2 mariadb 10.2.40, cent 7.9

php 7.1.11 mariadb 10.1.38, cent 7.6

I have tried this in 3 different browsers.  This is not a browser issue.

The server that is working properly has a pop-up window for creating a new db user.

The other 2 servers that are not working does not have a pop up window.

I tried to report a bug.   But bug link was not working.  Today the bug page is back in action.  I will report the bug there.   Release 1094 had a supposed fix for create db user.  So this is obviously a problem,......that has not been completely fixed.
